The primary thing is sanitation. Back then they lack the most basic knowledge of sanitation. Just by washing one's hands can cut 90% of any infection. Back then doctor's don't even boil their instruments. Technically there are no doctors. If you get sick, they may pray for you, but that's about it.
Also, today's separate of food sources and end products means animal-passed diseases are much harder to spread, not like back then where every other backyard had pigs and chickens and whatnot. Insect-passed diseases, on the other hand, are a bit harder to control.
