As a pregnant wife 5 months along, I'm totally qualified to answer this question. And the answer is, housework, housework, housework! My husband gave me a gift certitifate to a maid service and it's the best thing he's ever given me (except our baby, of course).
I couldn't care less about back rubs or foot rubs at this stage of the game - I just don't want to have to scrub the bathtub or clean the windows. The extra 15-20 pounds I'm carrying around makes everything a lot more difficult, and heavy chores like that are real work for me now.
If you can't afford a service, do it yourself. Toss in a little sympathy when she needs it, and you're golden.

Get some good foot balm, and give her some good leisurely foot massages, it will give her an opportunity to get her feet up, and feel pampered. Rub her back for her, especially her lower back. Take a genuine interest in her and the baby, such as reading to the baby when you are both laying in bed. I can suggest "The Good Earth" which i read to both of my children before they were born. The baby can hear the sound of your voice, and when the baby is born it will recognize you by your voice, and your wife will love you for it. sign up for some birthing classes, and attend with your wife, when the time comes it will be a great comfort to her to have such a great husband as a coach. Help her to prepare a "Off to the Hospital" bag, and help her prepare a nursery.

Well I have to say it is very sweet of you to ask. I am actually 5 months myself. I think a nice warm (but not hot because it can harm the baby) bath with candles would be nice. Back messages, love letters, if she is interested maybe a little romance but do stuff that makes her feel really sexy like you love everything about her growing body, she might be interested in a nice dinner in or out, flowers, there are so many ideas....

My husband rubs lotion on my belly at night. It's very soothing and it helps me believe I'm preventing stretch marks lol. Oh, and when she starts getting bigger, doing little things around the house like taking the clothes out of the dryer so she doesn't have to bend over or putting the dishes away on the high shelves is a nice way to show her that you're concerned about her comfort. :) Btw, congrats on being a good hubbie!

Ask her if she would like a foot rub and leg or back rub. If you are up to it, maybe all three. My husband was very attentive when I was pregnant, it helped a lot knowing he cared. When you are pregnant your hormones are all out of wack, so having your partner do nice things for you makes you feel a little less like you are so out of place. I felt like a beached whale. This is very kind of you to think of your wife when she needs you the most.

If your wife asks for something get it for her and the biggest thing and the think i was actually hopeing for but never happened was keep the stress off ur wife. If you see ur wife wants a drink or something to eat get it for her open the door for her. Little things actually mean alot so dont think of just the big things.

There are a lot of nice things you can do like massages, pedicures, etc.
If you REALLY want to do something nice, clean the house. Do the laundry. Do the grocery shopping. Clean out a closet where she can start to store the baby supplies. Trust me, she'll be really appreciative!

The best thing you can do is to assume the title of "Subservient House Wench," meaning that you are officially the laundry, cooking, cleaning, and errand man. Backrubs and foot massages are great, but if she needs to get up and clean afterwards, they won't do any good at all.

run her a bath with some relaxing lavendar bath. (you could help out and shave her legs for her. my bloke does that for me as i find it difficult with backache). give her a gentle massage when she gets out using a gentle moisturising oil or cream.
a meal out before she gets to exhausted would be great maybe a while before you can do that again stress free.
any thing you can do she'll love even little things like flowers any thing that will make her feel gorgeous and relaxed.

Draw her a warm bath complete with rose petals and candles.
Make her dinner...
Go shopping for baby items with her
Do a few chores for her.
Do anything to make her feel special.
