Middle-Earth is my favorite, followed by Narnia, Faurun from Forgotten Realms, Oerth from Greyhawk, Krynn from Dragonlance, and the Ravenloft demiplane. Usually I like to run Middle-Earth games using the Dungeons and Dragons rules, setting them in the years between Bilbo retiring to Rivendell and Frodo leaving Hobbiton.

The world in which David Eddings set the Belgariad and the Mallorean. He doesn't name the world or planet, but the books steal massively from the best of Roman, Viking, European and Egyptian histories. It's idealistic, romantic, witty - and there's magic! The perfect fantasy world.

You should read steven Erikson's Malazan book of the fallen series it is sooo detailed and consistent that it makes middle earth look like tele tubby land.
