Normally, I'd say "Dr. and Mrs. John Smith" But what should I do if both people are doctors?

In my opinion, since a Christmas card is informal, I would not put titles at all. I would Say Mary and John Smith and Family.

Whoever said Dr. Smith and wife... duh.
Why not Dr. Smith and husband??
I can't believe how non-progressive some people are.
BTW - the accepted address is "Drs. Jane and John Smith."

Yes, Drs. John and Jane Smith, or Doctors John and Jane Smith to be a bit more formal.

For a Christmas Card I would use John and Mary Smith or Mr and Mrs John Smith

So far some of the others that have given you their opinions as to say Drs. are right. Since both of them are doctors they both deserve the courtesy of that being recognized. Have a Merry Christmas.
