some people think you don't lose your virginity until you 'know what your doing' in which case, the hymen could be broken during sex.

Many girls and teens tear or otherwise dilate their hymen while participating in sports like bicycling, horseback riding, gymnastics or inserting tampons, or while masturbating. A girl may not even know this has occurred, since there may be little or no blood or pain involved when this happens. The tissues of the vulva are generally very thin and delicate prior to puberty. The presence or absence of a hymen in no way indicates whether or not a female is a virgin. * You are a virgin until you have sexual intercourse.*

It could first of all have been nonexistent in the first place or could have been very small. It could be stretched by sports activities especially gymnastics, bicycle riding and horseback riding and by use of tampons. Very few modern women have a hymen that is an issue by the time they are ready to engage in sexual activity.

Any injury to the area can cause that. There are many explanations. The worst one of course would be child sexual abuse unknown to the child. (They may not remember it).
