The input shaft of the transmission could be very dirty and not allowing the clutch disk to slide. Very hard to clean without removing the transmission.

it could be that little pin that stops fluid from going back into the reservoir when the pedal is pressed, mabey it is cloged and when you press the pedal the fluid just shoots back into the reservoir, so are you absolutely sure you have that same slave cylinder travel. my only other guess is the input shaft splines are very rusty and the clutch disc is binding and staying jammed against the flywheel, this could cause some clutch dragging and make the gears grind when shifting, but the clutch will not stay completly engaged if this is the problem.

Have you tried holding the pedal down and watch the slave and see if there is any bleed off ?? If so it is master or a leak. It seems a leak would be obvious a master leak would be internal and show no leak. The master could also be an intermitent problem as you describe.
