How do I efficiently run my wood fireplace in my home?

What do I do with the damper, open it fully or only part way? And do I leave the doors open to get more heat out of it or just use the screen that covers it?

insert will help, also glass doors on front help since the doors will radiate heat out after fire has gone down. close damper when not using the fireplace to keep stuff from coming down in to house. open 1/2 when starting fire and open fully when fire is going full force to let out all the smoke, I know heat will escape as well, but least your house won't fill up with smoke.
