1) Claudio Reyan, played for both Rangers and Manchester city, and has played in both the Old Firm and the Manchester City derbys.
2)Claude Makelele won the la liga with Real Madrid and won the english premiership with Chelsea!
Giovanni van Bronkhorst won the la liga with Barcelona and the english premiership with Arsenal.
3)Clarence Seedorf, he won the UEFA champions league with Ajax, Real Madrid and AC Milan.
4)Dietmar Hamann won those titles with Bayern Munich and Liverpool.
5)Marcel Desailly played in the French,Italian,English and the Qatari leagues, and he won the worldcup with France (1998) and the FA cup with Chelsea season 1999/2000!!!!!!!!

Assuming I've read the question correctly, a correct answer to 5) is Juninho Paulista, who won the WC with Brazil, league cup with Boro, and played in England, Brazil, Scotland, and Spain.
