It depends on the case.

In the nominative case, 'Ihr' is the second person plural informal.

In the accusative case, 'Ihr(e)' is the third person plural or second person formal, depending on gender.

In the possessive pronoun, 'ihr' mean her. eg. ihr Hund (her dog), ihre Katze (her cat).

If referring to the second person formal, you must have a capital I.

if it is used in a singular context then it is formal
if it is plural then it doesnt matter
just look at the verbs in the sentence that will usuallly help
