No, not exact. You can usually narrow it down to the city or town they're in based by the servers that a tracert would show, but that's about it. You can give this site a shot...

Definitely it can be traced.
Even if u are behind a firewall packet tracers give u the IP of the firewall and ur IP address. The firewall will be tarceable through your ISP and then they can hit your pc. Tracing of IP is no big deal if someone has the authority to do it.

Not from an IP Address. If you've got a wireless connection and are scanning the area for other wifi hotspots, and you have a GPS device, you can see the physical location of the wifi hotspots.

yes. You can determine the ISP or the organization but you need help from the ISP for that person or the organization that maintains the firewall to pinpoint the PC, as it might be a dynamic IP.

Not necessarily. I know for a fact that, if you were to look up my IP, it'll give you the name of some city I've never even heard of.
