Nikolai Andrianov holds the record for most medals won by a man (15 medals total). Larysa Latynina holds the record for the most medals won by a woman (18 medals total).

Tough question with not enough detail to it. Do you mean which country or which individual? Gold medal only or all medals combined? Winter olympics or Summer olympics or all olympic competitons in history? The simple practice of awarding medals didn't even begin until 1904 but the olympics go back to ancient Greece. Prior to that they assigned places like first place, second, place etc. So even knowing the most medals won would not indicate the winningest person or nation.

Larisa Siemjonovna Latynina of SOVIET UNION has won the most medal in the Summer Olympics history, winning 18 medals in total from 1956 to 1964 (9 golds, 5 silver & 4 bronze), all of which she won in artistic gymnastics event.
