Low Metabolism / Thyroid Problems
She probably has a "low metabolism." That means her Thyroid is not acting properly, at least for a regular person. A low metabolism can be normal for some people, as a high metabolish is for some people, but for alot of people it means certain hormones are not being produced correctly.
So, the 'only' way to know which she is, will be to take her to an Endocrinologist or regular doctor and have a thyroid test. With a low metabolism her body will not want to burn fat to spend it as energy, it will instead horde the fat and keep her tired and listless.

Then there is Cushing's Disease, a tumor of the Pineal Gland, very rare, that cause's people (mainly women) to gain weight in the stomach, chest and facial area. The legs, arms, etc do not gain weight. Cushing's syndrome is the name for what happens when your body makes too much cortisol. Cortisol is also sometimes called a "stress hormone" and is a natural steroid hormone that's like the "cortisone" in some medicines. Cushing's syndrome can be caused by the body making too much cortisol or it can be caused by taking too much cortisone-like drugs.
# Fat deposits can form around your stomach and upper back, but arms and legs stay thin and don't usually get fatter.
# Your skin gets thinner and is easily bruised.
# Cuts, scratches and insect bites take a long time to heal.
# Pink or purple stretch marks may form on your skin.

# You may feel depressed.
# Women usually have irregular menstrual periods and may grow thick or more visible body hair.

The most common ulcer symptom is a gnawing or burning pain in the abdomen between the breastbone and the navel. The pain often occurs between meals and in the early hours of the morning. It may last from a few minutes to a few hours and may be relieved by eating or by taking antacids.
Less common ulcer symptoms include nausea, vomiting, and loss of appetite and weight. Bleeding from ulcers may occur in the stomach and duodenum. Sometimes people are unaware that they have a bleeding ulcer, because blood loss is slow and blood may not be obvious in the stool. These people may feel tired and weak. If the bleeding is heavy, blood will appear in vomit or stool. Stool containing blood appears tarry or black.

A stomach ulcer is a place in the stomach where the skin has been worn down some. The area is no longer protected so stomach acid is getting into the raw skin. Antibiotics to kill h. pylori and some antacid will fix this. A stomach hernia is where part of the stomach pushes up past the diaphragm (the skin that separates the upper and lower parts of the abdomen) into the cavity where your heart and lungs are. This requires surgery.
As far as her weight, it could be genetic. She could also have a hormone problem, like PCOS (poly-cystic ovarian syndrome) or hypothyroidism. She can go to the doctor to have her blood checked for a number of problems and he can also advise a diet plan. Hope this helps!
