The "secret" ingredient is chosen from a list of five possible ingredients that are given to the chefs prior to the filming of the show. While they don't generally know the actual one that will be used, the time they are given to plan certainly does not make it a surprise.
The link below deals with Iron Chef America, but I've read (can't find the link) that the original (Fuji TV) Iron Chefs and Challengers were given a list of possible secret ingredients too.

i agree with you. i mean, how can they come up with 5 dishes on the spur of the moment. and what about their sous chefs? how do they know what to prepare without consulting with the chefs? sorry that i have more ?'s than answers. i would like to know too.

I don't think it'd be all that hard if your whole life revolved around cooking. With all of the knowledge they've gained, they probably know what ingredients go with what flavors.
