You need to give us a bit more info about what you are doing now, where you are interested in a PhD program, etc.
If you are interested in pursuing a PhD in the US then you need a Bachelor's degree at the minimum and in some cases you might also need a Master's degree (although many PhD programs will grant you a Master's on the way to the PhD.
You will have to take the GRE and fill out an application that will also include a personal statement. You will also need to send in official copies of your transcript and get letters of recommendation.

find the university you wish to attend. I have rarely found a university website that does not have an easy method of locating a link to academics and enrollment. You will need to find the application to the university, fill it out and send it in (usually with a fee). They will review your previous scholarly achievements and you will be accepted or rejected. Once you are admitted to the program, you may have to complete some prerequisites, but you will most likely have an advisor. Go into the program knowing what you want to study in your quest for a Ph. D.

You have to get your undergraduate degree first. Then you apply to a doctoral program. The process is very selective, and programs often emphasize undergraduate grades.
