there are many different types of hackamores on the market but most of them have the same size nose piece that fits most horses but if your horse has a smaller nose most bit sites do custom orders for the size of the nose piece I have hada few personally made for me and i have cut a few down by cutting the rope and remelting it really just depends on your horses nose size and also go with the rope nose incase you do ever need to make it smaller and they do make smaller hackamores but you usually have to order them from your tack supplier not a huge request for them

It's not really the size, you can adjust the chin strap to fit. The difference you might want to make attention to is the shank length. The longer the more stopping leverage you'll have. Most are pretty standard though.
