If you're talking about physical injuries, then Owen Hart's death is the most tragic moment.
Otherwise, the worst moment in WWE history was when Vince McMahon decided to enter the ring. He destroyed any respect for the performance art angle of professional wrestling.

and then jerry lawler says in wwe magazine that he does not regret finishin the show that was so damm right bad on owen hart but the worst moment has to be when eddie died that was and the guy or girl that crying over a guy hu died eddie ant just a guy he is the best man in wwe VIVA LA RAZA and if dont know already piper has got cancer

As far as personal goes, Owen's death was a tragic one and probably one of the worst moments in wwe history. It was hard to see something like that happen to a very talented individual!
As far as business, the worst moment for the WWE would be when they bought out all of their competition! The storylines grew weak and stale.

The deaths of Owen Hart, Eddie Guerrero, Chris Benoit. the attitude even as Mae youthful gave delivery to a hand.even as Triple H had sex with Katie Vick, the completed storyline became ill. Hulk Hogan even as he had the Mr united statesa. mask, all of us knew it became him. Mr Mcmahon demise contained in the limo.

What happened to Lita at Survivor Series was just wrong, Making her go out like that was just out of order but as she said on her website she had to remain professional even though thats not how she wanted to end her career.
