The exaust pipe is a straight shot venting to the outside... can it be insulated, would that help? Looking from the outside, it is just a PVC pipe with no cover, just a grate.

Should be a flapper in the cover. You can buy something called a dryer vent hood and put it on the end of the pipe. That will stop the cold air from comming in.

Get a cover for the outside, they have flaps that open when the fan is on and close when it is off. They are sold at any home improvement store (Home Depot)

a quick fix is to plug the vent, there should be a flapper or valve to prevent the back draft. sometimes they are just vented into the attic

You need a flapper cover instead of grate cover.
