Could be anything. The leader of one army would send a challenge the leader of the other army. The challenge would specify the manner of one-to-one engagement, as well as whether the leader was expected to meet the challenge personally. The other leader would either accept the challenge or lead his army to the battle.

Off the top of my head, I'd say, the most common arrangement was heavy-infantry gear (helmet, body armor, shield, spear, and short sword), but combat chariots, cavalry gear, and even hand-to-hand combat weren't unheard of, either.
