Go to your local post office and pick up an application, or better yet, go to the State Department website (travel.state.gov/passport) and dowload the application.
Fill out the app, attach appropriate pictures and money and send it in! Viola... you have a passport (allow 60 days for processing)

Passports are relatively easy to get; your local Post Office is the place to go - you can apply there and even get your passport photos taken there.

go to the local post office. They have application forms and you can pay at the post office for the processing, etc. They will even take your photograph for you, or you can go to kinkos and they will take passport photos.
The post office has all of the paperwork, fees, etc.

GO TO THE COURT HOUSE IN YOUR COUNTY AND APPLY FOR ONE. YOU CAN ALSO DO IT AT THE POST OFFICE.
