Rule #1: it takes two to tango. LOL.

This question is a bit too general. What kind of Tango? Argentine or Social or International or some other kind? Have you been dancing? How long have you been dancing? I don't want to start telling you all the basics if you know them, and I don't want to give you boring details if all you want is basic ideas.

General rule (for non-competitive dancing) - the girl defines the distance between the partners she's comfortable with. For competitive - your instructor tells you what that disctance should be, and then girl defines the distance with that info in mind.

Depends on whether you're dancing Argentine tango, International style, or American style. The character of Argentine is somewhat different from the formal ballroom styles, where a lack of rise and fall and the presence of staccato motion are a couple of things to keep in mind.
The business about "breaking hold more than once" was specific to that TV show. In International style, one never breaks hold. In American style, it's allowed, and underarm turns and shadow position are quite common, though one wouldn't wish to devolve into two people doing solos--it is, after all, a partner dance.
