Yes, he died. The Minister and Dumbledore were shouting at each other in his office, and they even said a man had died. He was murdered by "Alastor (Mad-Eye) Moody" though it was really Barty Crouch Jr. having taken a polyjuice potion.

he was killed. in the book it is more clear.
at first harry is walking with viktor krum discussing hermione, and then barty crouch sr. pops out of the forbidden forest and is mumbling Crazy. he seems to be under a spell, and then fights hard against it and regains consciousness and says "I need Dumbledore!" and says incoherent things about Voldemort being back, etc. then he goes back to acting under a spell. so harry leaves viktor w/ barty and goes for help. he tells snape and eventually dumbledore and they go down to find that viktor was stunned and barty sr is missing.
barty crouch jr. eventually confesses (via veritaserum) that he had seen his father on the hogwarts grounds on the Marauder's map he took from harry. he (of course, looking like Moody), went down, found his father and killed him w/ some killing curse. then he turned his father into a Bone and buried him in the ground.

He was killed by his son Barty Jr, who used a potion to disguise himself as Mad-eye moody
