I would say its a plastic surgeon in a private Hollywood clinic. No reflection on expertise...I just think its where the money is.

The surgeon that has to pay the most for malpractice/liability insurance. This is most likely a Cardiologist based in the Northeastern United States. Second would be his/her Anaesthesiologist.
Physicians in US states with high amounts of malpractice claims pay exorbitant malpractice insurance fees, largely due to legislative climates that easily allow frivolous lawsuits to be filed. The northeastern states have been slow to implement tort reform, which will provide stricter checks and balances against patients filing frivolous lawsuits.
Cardiologists are considered a high-risk specialty. Life in the US tends to be one of stress and excess, therefore a high risk for cardiac disease. Cardiologists get a lot of work, and therefore have the highest statistics. Less invasive technology is helping.
Unlike other countries, healthcare in the US is 100% private. This means that everyone in the healthcare transaction is responsible for providing and financing their own insurance. Cardiologists and Anaesthesiologists have to charge the most money to cover their malpractice insurance, while patients' insurance wants to pay the physicians less and less. The physician gets squeezed in the declining spread.
One would think plastic surgery is a higher liability, but typically in the US it is less so. Most plastic surgery is elective, and not paid by private insurance. Therefore, plastic surgeons have you "sign your life away" when it comes to liability.
Outside of surgeons, US hospitals have some of the true highest liability, especially Intensive/Critical care.

Concurring with the majority, I would take a guess at the plastic surgeon. Sucking fat out of rich women's chin is an expensive business.
