Most of the lava is in plasma state. Neither liquid nor solid. At very high temperature, electrons in outer orbit of any atom gain enough energy to get out of influential field of atom and become free electron. This is called process of ionization. Plasma is a state ionized state of matter

Molten (liquid) rock is magma when it's still inside the earth. Once it come out of the earth, it's lava... Once it hardens and stops flowing, it becomes a solid.

When it is heated back up yes...Its like ice. When ice is frozen it is a solid, but you can easily turn it back into a liquid by heating it back up.
