The only time I heard the word muggins attached to this phrase was in the movie Elf. I could be wrong but I think the word muggins was just added for humor effect. In an episode of I love Lucy, the term "cotton headed ninny" was used without the word muggins. Again I repeat I could be wrong, but it sounds like it was once probably used as a kind of humorous racial slur. As in cotton headed picaninny.
