When I was pregnant my OB GYN had prescribed 1 mg ativan for my panic attacks. I literally take one maybe a week if that only when I have a really bad panic attack. I forgot to ask my doc about taking them while breastfeeding and now its Saturday night and I don't want to wait until Monday to find out.

From Medications & Mother's Milk by Dr. Hale -

He says that the levels of Lorazepam (Ativan) secreted into milk would be "clinically insignificant under most conditions."

He goes on to say, "The benzodiazepine family as a rule are not ideal for breastfeeding mothers due to relatively long half-lives and the development of dependence. However, it is apparent that the shorter-acting benzodiazepines are safest during lactation provided their use is short-term or intermittent, low dose, and after the first week of life."

Under Pediatric Concerns he says, "None reported via milk but observe for sedation."

This is a question for your doc. The issue with ativan is that you have to weigh the potential harm (I'm not sure how harmful) to the child against the harm the mother would suffer without it. I took ativan while pregnant multiple times under the care of the leading womens health/PPD doctor in the country. I also was given it in the hospital just before labor. Sometimes the benefits far outweigh the possible harm that could be conveyed. The other thing is with some medicines they are crucial to your health even though they would harm the baby during breastfeeding. That's where you have to make your personal choice of whether or not you think the baby would benefit more from a crazy mommy and breastmilk or a sane mommy and formula. I went with sane mommy and formula because the mental health issues in my family are so strong that I didn't want to risk the chance that I might endanger my daughter. You really should discuss all of your concerns with your doctors. Good luck!

Ativan is a benzodiazipine in the same family as valium and xanax. It treats SYMPTOMS of anxiety and panic. Many people find this class of drugs quite helpful for panic. It is, however, a drug that you can build a tolerance for and for some people can be addictive. They can also be used on a PRN or as needed basis. You should discuss this with your doctor. If you would like to try going off, you need to do it with their direction and never cold turkey. It is always hoped that while taking drugs such as Ativan that you will learn other ways to handle the panic, usually through Cognitive Behavior Therapy. Good luck with your panic.

If you take it while breastfeeding your baby will also be affected by it. If you really need it then you can take it and supplement with formula for 24 hours and make sure to pump your milk during regular feeding times and discard it.

I'm not sure about this one - any hospital around you should have a nurse line that you can call.

No you cannot. Benzodiazepines become concentrated in breastmilk.
