You should have good grades that show you try. Another factor would be having a rigorous course selection that shows what your interests are. Make sure you have a passion for what you do, and don't try to be well-rounded. Do what you like to do. If that means concentrating on math and science, then start a math and science club at your school. Too often, there are kids that participate in everything and wear themselves out.
Good colleges want a motivated student who challenges themselves in different ways each time they can. This is why you see so many athletes and dancer and all-around involved kids getting into good colleges. They want to be the best, and they are willing to work hard for it.
Also, try to help out your community and find a cause that you are willing to help with. Everyone likes to volunteer if they can find something to relate to, and it really makes you look like a good, down-to-earth person.

Good grades is the most important. With good grades you can receive various honors like NHS membership, State Scholar, scholarships which all look great on your application and aren't too time consuming. Try to be in the top 10% of your class and take challenging classes (AP classes) your junior and senior year. Kick major butt on the SATs and ACTs, especially if your GPA is so-so. Ask teachers for letter of recommendations well in advance and before you ask get to know those teachers better. The more personal of a letter they can write, the better. One or two extracurriculars should be enough, too many and your grades will suffer.
