There's a great one in a Bond film- Bond drives a car on it's side through an alley. When the editors came to put the film together, they realised that he drove out the opposite way he went in, and had to shoot an additional scene where Bond turns the car onto it's other wheels. If that had been possible, he could have driven through the alley normally! That makes me laugh every time I see it!

The best one is the fight scene between Little John and Robin Hood in "Prince of Thieves": they seem to be dry in some shots, wet in others, when somebody puts his right hand down to help the other up, in the next shot it's their left hand. It's a total mess.
Another good one is in "A Fish Called Wanda" - where John Cleese is dancing around naked and the family walk in and he grabs their picture to cover his bits. In the shots from the front he is indeed naked, but in the shots from the back he's wearing trousers.
