It depends on a number of factors. A lesson or objective may be one that one particular person finds difficult to understand and others may not think it is difficult. So the lesson may not be hard for everyone just some.
On the other hand an instructor may present lessons in a variety ways that may make it easier for everyone to grasp to the best of their ability.
There are some teachers that may attempt to challenge students to cause them to stretch and grow mentally. Just making things easy for students does not necessarily help them in the long run.

i can think of 5 factors that can make a lesson hard.
1) the complexity of the lesson itself
2) the quality of the text (if any) that presents the lesson
3) the quality of the teacher's presentation of the lesson
4) the skill of the teacher presenting the lesson
5) the skill of the student studying the lesson
i don't mean to insult you, but you shouldn't assume that the problem isn't you.

Good question. I think it's the teacher. Most lessons, no matter what the subject are presented in the books in dry, black & white styles, so it kind of teaches itself. But some teachers complicate things by injecting personal opinion, abstract examples, or just poor communication skills. :)

More often than not I think it's the teacher making things hard by not properly explaining things. I had a Geometry teacher that couldn't explain things to save his life. With a different teacher I was at least able to pass the class.

I think the teacher, because teachers have different ways to teaching, so it could be hard for the students to comprehend what the teacher is trying to get to you.
