I'm assuming that since you are posting this in the "Higher Education (University +)" category, you are planning on writing a recommendation letter to a college.
About college recommendation letters: I am a college student who works in the admissions office and you wouldn't believe the recommendation letters that come in. Some have poor spelling, some don't mention the student's name until the last paragraph (we had one who didn't mention it at all!), some don't include the student's area (It's required for application in my school. If the office doesn't know the student's intended area, then they don't know what faculty member to give the letter to), and some people go on for paragraphs about themselves, instead of the student.
Also, if you are going to say something about the student, then you should back it up. It's easy to say: "Jane Smith is a leader in her school." But why is she a leader? "As Class President she has motivated the students to take pride in their school. She recently implemented a peer tutoring program and has also organized drug and DWI prevention discussion groups." Now the reader knows exactly why they should accept Jane Smith.
