"I wish I knew what she was thinking when she looks and smiles at me."

I wish I knew what she's thinking when she looks at me and smiles. That's not a bad alternative. There's nothing really wrong with yours. Some people will use was (past tense) instead of is, depending on the situation.

the word "knew" is past tense there for you should use the word "was" .
