well in the world of a beautition, A " Back, Sack and crack" is commonly known as waxing the Back (back) sack ( around the groin) crack ( hairy bummed people).

it's not in the hairdressing world it's in the waxing world. A man would ask for a Back, Sac and crack e.g - a r s e, balls and back

Extremely painful for the guy getting it.
It's the removal of hair from the testicles (Sack), the Back, and the anus (Crack).

It is when hair is waxed form a man's back, bottom (crack) and testicles (sack) - sounds painful to me!
