Having worked as a newspaper reporter, I can confirm that the above thoughts are accurate. I can add, though, that in some cases, newspaper will have reporters who spend at least part of their time writing obituaries much as they would write an article, if the person who has passed was prominent. In some cases, if the person is well known, the newspaper people will take it upon themselves to do the article via talking to relevant people, doing research via the Net and other sources.
In some cases, newspapers might not be aware that someone who passed away had, say, been a champion tennis player in the 1940s, earned uncommonly high-level military medals in World War II, Korea, etc., written a best-selling book decades ago. In cases like that people who knew or are related to the person who passed away will often bring that sort of information to the attention of the newspaper.
Newspapers have been giving more and more attention to those sorts of situations because they realize it provides a valued and appreciated service, that readers often find it interesting and worthwhile to learn of people's lives and accomplishments.

Usually the funeral homes makes arrangement with a local printing shop. The graphic artist do them.

and then direct them to which newspaper(s) you want it to appear in - most are now charging
