Yes, certainly. Every packet sent to or from your PC passes through their routers. If they really care, it is trivial for them to snoop on all of your traffic. Even when you use encryption (such as SSL, for secure web pages), only the *payload* (the contents) of the data packet is encrypted -- the "header", which contains source and destination address information, is in plaintext. The "destination" address of packets outbound from your PC unambiguously identify the website you're visiting.

Yes, every connection you make is routed through your ISP. In many cases, however, it is not feasible for them to record this information for any longer than a few hours to a day. Check with your ISP for how long they keep logs.
