It probably does hurt a bit. He has a high pain tolerance. Perhaps he practices meditation to get his mind off of the pain. Also, He probably kept his abdominal muscles flexed to protect his organs and spine.

There is a condition (that is rare) where people cannot feel any physical pain. Maybe this guy has that condition?
