If you are living and working in Spain and therefore paying social security then you will be entitled to the same health care in Spain as Spanish nationals.

The E111 card died at the begining of 2006! It was replaced with the EH1C (a plastic credit card) which does the same job. Easy to apply for on-line (mine arrived in 3 days!) Check out: two websites - Spanish Property Today and Spanish Property for Sale

no if you have an e111 you will get reciprocol medical care as you would in the UK
the Spanish system used to be free but they charge for it if you are a non resident i pay around €95 per month for my family with Catsulut.
but beware it only covers certain costs
