I would say that most decent jewellers sell them, I don't smoke myself but my Mum has bought one for a friend before. Zippo do very pretty personalised ones but they only have a small selection of names, so your name would have to be common. Another option is to buy a metal cigarette lighter and then take it to your local jewellers and they will ingrave your name, initials or whatever you want for you there, it's not very expensive either!

Buy an ordinary one then etch your name onto the side with a compass/something which is similarly sharp and pointy.
It will save you money AND look artistic
