It costs money, but I find AI Roboform to be the most functional and easy to use. I tried other free ones but they are not use friendly like Roboform. Spend the money it's woth it.

Keepass Password Safe http://keepass.sourceforge.net/ is great! AES and Twofish encryptions (VERY strong). Open Source and free to use. For Win, Linux and Mac OS X. Less than 1 MB. Easy to sort passwords in groups. Easy drag and drop passwords to different windows. In all a great piece of software.
Worth mentioning is also Key Wallet http://www.keywallet.com/ and Locknote http://locknote.steganos.com/. Locknote does not need to install and can be carried on your USB-pin or other memory card.

Hello - I've been using RoboForm for several years and it does a great job. It's free for a small number of passwords and only a small cost for many. It also does many other things - fills in basic info, etc.
However, I would still keep a separate list of all User ID's and passwords.
