I would try and hire one with your holiday company your flying or booked with. Take your drivers licence!

Use a reputable company like Avis/Hertz etc. I would prebook your vehicle on the internet first, so you can just pick it up when you get there. Watch out for the insurance. You need to make sure you are completely covered for everything as just bringing it back with a scratch on can result in a hefty bill. Always check the car before you drive off in it, note down any scratches, damage etc anything you do not think right and tell the people as when you return it they can bill you for it thinking that you have done it. Check the petrol before you leave, if the tank is full then you have to bring it back full.

Yeh, get one. If you can afford the extra cash its far better than using public transport. You can have the freedom to explore whereever you're going and the time to see more.
Book it before you go (cheaper). Go to www.autoreservations.com, they use all the top dealers at discount prices.
And even though I hate insurance, I'd advice getting the full damage waiver, there's some crazy drivers out there!!

Depends which country you're going to. From experience:
1. Read the small print (Things like "unlimited mileage up to 100KM" is a favourite)
2. Ask tour rep.
3. Use a reputable source.

It's going to be fine as long as you are well-versed with local traffic rules and roadmaps. Carry your American driving license and travel insurance papers and if you are driving in Asia, make sure you acclimate to the road conditions there.
