Tough question. Grandmothers.. that's always hard.
Well, grandmothers always like to brag about their grandchildren (and for some reason they love them more than their own children), so if you're the kind of people that likes to make things (art crafts, photography... anything at all) I think that would be a great idea. If you're not, I think the best gift would be somthing for the house, since she spends a lot of time there. It doesn't have to be big, it's the thought that counts.
Do you know if she like to listen to music ? Any favorite old singer ? Maybe you can get her his/her record/CD...
Another idea is a picture of all her grandchildren, I really think she'll like that (I'm thinking of my grandmother). Black and white or colored, both are beatiful.

My mom has a similar routine, and lately I've started giving her gifts that encourage her to go outside for even just a few minutes a day. I've planted a flower bed for her, and put up a bird bath near the front of the house. She loved these gifts, because they were things that she would never have done for herself, mainly because she has a hard time gardening.
If gardening is a bad idea for your area, or her particular home, how about a beautiful potted plant? Get a nice decorative planter, and put something pretty in it.

Pay for something like one of her medicines that is affordable for you, buy a nice frame and put you or your family picture in it, pay one of her utility bills, take her out to eat, or spend the day cleaning her house from top to bottom, do some yard work and what worked best on my mom was a dozen white roses because she would say "Why do I need roses at my funeral where I can not see them I want them now!" My mom is now passed but these are some of the best gifts me and my daughters ever gave her & we knew by how she would brag to her freinds how one of us spent the day with her.

I didn't know what to get my wife's grandma either so i got her a picture frame and then i put a picture in it of her small dog that had died. It was a small frame that sits on her night stand(5x7). She cried when she opened the present and i felt bad, but she said it was the best gift of the season, It doesn't have to be a pet. I t could be of a building from her past, old home, church, deceased family member,etc, Something that reminds her of happy memories.

Tough one.
Dont want to be all morbid on you, but my grandma passed away last year. I really wished I had asked her more about her life.
I saw in a bookstore the other day a book for grandparents that they fill out. Its got all sorts of questions in it. eg. what was your favourite toy when you were a kid? What did you want to be when you grew up?
That sort of thing.
I really wish I could have gotten my Grandma to fill it out before she died.
Im sure Borders or a similar store would have one of these books. Itll be something to treasure and hand down to your kids.

i'm very satisfied to take heed to that somebody takes the time to locate something valuable for their grandparent being a grandma myself. in case you have a expertise alongside with writing or playing an tool you may write a poem to her or checklist your self playing music or making a music or making plans a picnic with only you and her. Or spend an afternoon at a Spa along with her, get facials, manni-pedis and or rub down and then do lunch. i desire this helps, sturdy luck

Stationary---cards, envelopes, stamps and a nice ink pen. Put them in a pretty gift bag for the holidays. She'll appreciate it, totally...

If she has good moter skills you could get her yarn and hooks and a book to teach her to knit or crochet(sp). Or a nice blanket or comforter. Or a lovely picture of you and your siblings/cousins.
I think something sentamental(sp) is always nice like a poem or a picture you drew/painted.

Have you thought of maybe some books on tape or CD? If she has some way to play them, they could provide her with lots of entertainment.
