Um, yes it does. Even if you have it up far enough, water still gets in. That's perfectly normal, just make sure that once you're done swimming or whatever that you immediately use a fresh tampon. You don't want any infections!

your tampon gets wet because the string is hanging out and getting wet...wicking the water up to the tampon. Once the string is wet and the tampon is wet...the blood is diluted and wicks out. The best way to avoid this is to push the string inside you. You will have to fish it out later, but if keeps you from getting a wet tampon.

It would have to, wouldn't it? However, unless it's quite heavy with blood, it won't dislodge. You should change it soon after swimming though...you don't want pool/lake/ocean water trapped up inside you for hours, along with your flow.
