There are two different types of forms you can change into. One is a "travel form" which does nothing but allow you to travel faster than walking. The other is a combat form which gives you different combat capabilities than if you were humanoid. You can stay in travel forms as long as you are traveling (you may get shifted back to humanoid under certain circumstances). You can stay in combat forms as long as you like (you can't communicate with NPC when shapeshifted so you may need to change back to turn in quests and such)

Druids get quests to learn how to shift to a particular form. Most of the quests involve teleporting to Moonglade and talking to the trainer up there. Each form has its own quest.

The druid gets his first shapeshift (the bear) at level 10. The bear is a combat form similar to the warrior, high armor and aggro generating abilities

The next is the sea lion at 16. The sea lion is a travel form which can swim without drowning. You can only transform into the sea lion when swimming and you shift back automatically if you leave the water

Next is the cat at about 22. The cat is a combat form similar to the rogue. Stealth and high Damage per Second (DPS) but low armor.

Next is the cheetah at 30. The cheetah is a travel form which allows you to run at a higher speed (30% higher). You can only shift to cheetah when outdoors and shift back to humanoid automatically if you enter a building.

Next is the dire bear at 40. The dire bear is a combat form similar to the bear. Higher armor (like a warrior who can now wear plate mail).

There is also the moonkin form which you earn from the talent tree. The moonkin is a combat form with higher armor (not as high as the bear) and some limited spellcasting ability (balance spells which do damage but you can't heal)

No. It's druids that transform into those. The change is for as long as you want. There are several forms. A bear form that has high AC and the ability to soak damage as a tank. A cat form that act much like a rogue. Another cat form that is a type of travel mode and a seal that can swim and breathe underwater.

Shamans have a sort of travel form where you transform into a wolf.

Shamans only get the wolf travel form - I think it's at level 20, but I'm not 100% certain. It follows the same rules as the druid cheetah form - only outside, but otherwise permanent until you turn it off.

the first shapeshift is the bear at lvl ten, then it probably goes in increments of 10 lvls.

You can transform for however long you want until you change back or die.
