You have to be invited by someone who is already in the mafia. i was invited to join them two years ago, I refused ! Once you are "in" the only way "out" is in a BOX with brass handles and a panel that allows people to see your face !

it's easy as 123 , first you have someone in the mafia to get you in. then you give a list of your family members. next you have fun for a few years, then get killed, or your family starts getting killed, and if you are lucky, you may spent the rest of your life in jail, but that's only if you are lucky.
