move all of rural America to Canada

Ultimately, it's an economic question, and because docs/hospitals can't make big bucks serving rural populations, the standard of care in rural locales will always lag behind the level of care available in large urban centers.
The only thing that will change that state of affairs is the introduction of a national health care system, with a guaranteed standard of health care for every citizen. Until we finally take that step (proposed by President Harry Truman nearly sixty years ago), any other action in the area of health care is just a band-aid.
I know many free-market conservatives will be skeptical, but it made an impression on me that my father, a Republican and a career business executive--the ultimate economic conservative--long ago concluded that national health care was the only realistic solution to the whole range of health care issues, including rural access.
Hey, the Brits and the Canadians have it, and I don't see their civilizations falling to dust.

Well first of all how does rural America differ from Urbanized America? This country has the best healthcare in the world in spite of the things that still go wrong. The real question is how can people find insurance they can afford that allows them to go to the doctor they choose and get the kind of treatment they choose?

Set up doctor's visits that aren't so expensive. I know that malpractice insurance is very expensive but some people don't even have the money to pay for office visits.

This is an excellent question. As a nurse, I wish I knew. We are spread pretty thin now, as it is and they keep piling on the work.

Better health and sex education, state- or federal-funded health coverage for all, free traveling clinics for vaccines, and so on.
