It seems to me that this is a relationship between two people. The singer seems to just do his thing while the other seems to be following him. But the other also seems to have problems and is kind of obsessed with the singer. The other person points out the singers flaws but still won't leave. Like the other person thinks he's "lost" and "contagious" but the singer is still curious as to why the other person still "follows to higher ground" and "drinks from his hand." And to me it seems as if the singer is just fed up and asks the other person, "if you don't need me or like me, then why don't you leave me alone (turn your head and spit me out).
