even if a diaper is dry it should be changed regularly. the constant humidity of the diaper being there causes irritation so you should change them every 2 -3 hours....even if it's dry.
making sure their bottom is good and dry before it goes in the diaper is good too.
avoid putting powder IN the diaper...when that gets wet it makes a gooey mess, and only makes it worse. rub powder or cream into the skin.
if your baby is irritated i wouldn't bath too long....moisture only irritates it further.

My grandmother used a couple of these, so they are tried and true!
Let him go bare bottom for 15 or 20 minutes several times a day.
Put some oatmeal in his bath water and swish it around his tush for 15 minutes.
Mix Vaseline and corn starch to make a paste and put it on the rash at diaper changes.
Pour milk of magnesia over the rash.
Apply warm cornstarch. (My grandmother called it "burnt cornstarch". She put it in a pan and put it in the oven and almost toasted it. She also did this with flour if she didn't have cornstarch. I think just warm is ok :)
When my kids were babies I used Penaten ointment. It was the only one I found that worked.
Avoid baby wipes. Use a soft cloth and warm water and pat his bottom - don't rub. No soap. Air dry or use the lowest setting of a blow dryer - no hard towels..
Vaseline is good as a prevention.
Make sure you use a good diaper that will wick the moisture away from his body.
Mix a tablespoon of olive oil with a few drops of water until creamy and apply it to the rash.

I can tell you the best way to prevent it is to use vaseline and desitine or balmex at EVERY diaper change. My daughter has never had a diaper rash.
Corn starch should help get rid of the one your babe has though.

The thing I do is at night (when their diaper will be on the longest), apply A&D regular ointment (NOT the zinc oxide stuff! Just the petrolatum) to the rash. This protects your baby's skin from his urine during the night. During the day I use Butt Paste (that's really what it's called!) It's more expensive, but it's 16% zinc oxide (the active ingredient), whereas the others are all 10-11.5%.

Are you nursing? Don't laugh, but rubbing breastmilk on his tushie and letting it dry off and airing out his bottom works great.
You may also want to change brands of diapers, try changing him more often and if you live in a warm area, bring him outside and let a little sun on his tushie -- not so much for sunburn though

I use Nivea hand lotion to prevent diaper rashes, its kinda expensive but havent had one since we started using it. Just get the liitle round tub of the regular stuff(not firming or anything!) and put a little on his butt every diaper change. and I think its called nystatin, it clears up rashes, but you might need a script for it.

make sure that baby is complete dry, in that area. use baby powder. im a huge fan of desitin. been using it for 33 years, not just for diaper rashes. i use it on my 3yr old. it works great for him. use alot in that area. u should change diaper as soon as it gets wet.

cornstarch is a really good remedy just use it like baby powder. another suggestion is letting him sit in the bathtub with baking soda in the water. it helps to dry out the rash. I did this with all of my kids and it worked well for them. good luck to you hope this helps

bag balm is best cure all i have ever come across, great for diaper rash, chapped hands, lips, dry skin, cuts and oh yeah, its intended use as a balm for cows udders/

my mother taught me cornstarch with my 1st child and to this day, i use it on my 2nd child. works awesome. Put the cornstarch in a grated cheese dispenser and tape the big holes. i've been doing it for 8 years...never failed
