If a guy has it, will it go away gradually with home treatment (bed rest and elevating the scrotal area, plenty of water, possibly ibuprofen, wearing scrotal supporter, or does a person have to take antibiotics to get rid of it ?

You should definitely see a physician because if what you are suffering from is actually epididymitis then it won't likely just 'clear-up.' Unless the cause is simple inflammation without the presence of bacteria, you'll probably need to be treated.

It's important to remember that the infection of the epididymis can lead to the formation of an epididymal abscess. Progression of the infection can lead to problems with your testicles -- epididymo-orchitis or a testicular abscess.

Sepsis is also a potential consequence of severe infection. Bilateral epididymitis can also result in sterility due to occlusion of the ductules from peritubular fibrosis.

Men with epididymitis secondary to a sexually transmitted disease have two to five times the risk of acquiring and transmitting the human immunodeficiency virus.
All sexual partners of patients with epididymitis secondary to a sexually transmitted disease need referrals to assure that they receive adequate testing and treatment.

You have to go to the doctor on this one. Two reasons:

1. There are other conditions that has similar symptoms requiring different approaches.

2. You may need some prescription medications, like antibiotics.

all of what you stated and antibiotics too
see your doctororor face the possibility of medical castration or sterility if the infection spreads

There is no "If" in this case every guy has a penis which means he has to have a epididymis.
That is you choice, and your risk don't take anti-biotics and you may have a larger penis comparatively.
