I would suggest NOT getting him clothes, unless you know FOR SURE that he has been wanting a certain shirt, pair of jeans, etc. Honestly, how much fun is it to get clothes? Everyone has their own style, and its entirely possible he wont like it. Plus, when you buy clothes as a gift, there is always the issue of finding the right size.
Since I don't know what he likes, I will just make some general suggestions:
-A DVD, video game, or CD that he has been wanting (or a different album from a band/singer you know he likes)
-A gift card to his favorite restaurant (I would use this as an addition to another great gift...getting only a gift card might be dissappointing)
-A bottle of his favorite cologne
-If he has hobbies (fishing, hunting, snowmobiling, skateboarding, waterskiing, etc) you could get him some relevant equipment/accesories

skip the clothes. It's too personal too soon.
Get him some music or pay for a dinner out with the two of you. You could buy him a DVD, too.
If you want to be unique, think back on conversations and try to get him something that fits what you both have experienced or talked about.
You could also buy him concert tickets.

I have some concerns about you having a 4 month old boy friend. But, they like things that light up or play music.

not sure what he's into but here are ideas...
-a photo of the two of you framed, maybe black and white
-a mixed CD
-some sort of entertainment, concert tickets, pay for a great night out
-book
-sweater
-game
-a watch or an ipod nano if you want to spend more
