Last episode od season three, she and the sisters use a power of three spell to kill a demonic assassin named Shax only to be caught on film and outed as witches.
Inthe hysteria that followed a wannabee witch gets rejected by the girls and wind up shooting Piper. Phoebe and Cole go to th Source to try and get him to use the Tempest to turn back time for a truce. Meanwhile. Prue loses it and starts hroughing cars and people around like they are nothing to get Piper to the hospital where Piper dies. Prue is about to die when Phoebe strikes a deal with the source to stay in the underworld so long as he turns back time and warns Prue and Piper about Shax.
Time is reversed but no warning is sent and both Prue and piper are severely injured. Leo, who is also down in the underworld at the time is unable to sense what has happened same as Phoebe. He arrives too late to save Prue but in time to save Piper. the first time Shax attacked when they originally vanquished him Leo was able to save them both, but he arrived too late this time. This is explained in the first episode of season four.

The demon Shax killed Prue. He was going after a Doctor (in the season finale) and Prue was caught, along with the Doctor (who also died in the episode) in Shax' energy bolts which came out of his hands.

The Source sent Shax (another creepy looking demon - why couldn't they all look like Cole?) to kill some doctor, and while the Charmed Ones were trying to protect the doctor, he killed him and Prue.
