no. you just need his information when you fill out the birth certificate. the only one that signs it where i am is the birth mother. legal documentation through the courts could be signed by the father...but in most cases only the mother's signature is needed on the birth certificate as long as the father's information is on the form.

You can put whatever last name you'd like; however, if he is going to be listed as the baby's father on the birth certificate then he has to be present and sign the certificate.

Not in Alabama, but inTennessee, I can't advise you on that. Does he even want to be involved in this child's life? Reason being, "there's a big chance he won't be present for the birth, I also don't know if he'll be able to get here soon after the birth". I say if he will be in the child's life, put his name down on the bc.

No, he does not have to sign. You can give the baby his last name with or without his blessing. If he wants to add it at a later time, he can sign an affidavit of paternity which after filed with the local court , he can then add his name to the birth certificate. This can be anytime after the birth, even up to years down the road.

There is NO WHERE for the father to sign on a birth certificate. If the babies last name (in Nevada anyway) differs from the mother's and you want the baby to have the father's last name you may have to fill out an "affidavit for paternity" (this has BOTH signatures notarized and states that he knows and agrees to you are using his last name and listing his SSN)

You can give the baby his last name, but if he does not sign the certificate, you can not hold him liable as the father. If you are concerned he may bail on supporting the kid, you may want to find a way to get it signed. No signature is the same as denial of paternity.

Yes, you can name the baby anything you want. Americo Jewarlious Jenkins if you wanted to (patent pending LOL) To add the father on the birth certificate he will need to be present.

Nope the name is totally up to you just make sure that he signs the birth certificate so its written in stone that he is the father just in case he tries any funny business.

No, in fact on my birth certificate there isn't even a place for the father's signature and I have my father's last name.
His name and info are on it but the only line for a signature is for mom. (granted I was born 30 years ago)
