Some of the worst covers I have ever heard have been:

Sheryl Crow - Sweet Child 'O Mine
Kid Rock - Feel Like Making Love
I don't know who did the cover but two horrible dance like versions of Listen To Your Heart (Roxette) and Heaven (Bryan Adams)
Limp Bizkit - Behind Blue Eyes

Some covers that I LOVE are:

Limp Bizkit - Faith
Ozzy Osbourne - So Tired, In My Life and What's Going On
Ugly Kid Joe - Cats In The Cradle

I like Off By One - Torn (Natalie Imbruglia cover)
Yellowcard - Everywhere (Michelle Branch)
The Starting Line - I'm Real (Jennifer Lopez)
Much better than the originals.

I really hated the covers Avril Lavigne did of Green Day - Basket Case (that songs been done to death) and Knocking on Heavens Door

The most successful UK cover song has got to be the Wet Wet Wet version of 'Love is all Around'. It was wierd because I saw Reg Presley sing it about six weeks before the Wet Wet Wet release, and although he wrote it, it completely ruined the momentum of the Troggs set.

Juliet Turner does a great version of Toxic by Britney Spears. I heard Director on the radio the other day murdering Maneater. It was shocking. And who can forget Mike Flowers Pops doing Wonderwall!!!!!

And who was it that did Behind Blue Eyes? limp bizkit or one of those guys. HOW can anyone think its a good idea to cover the who? Now I ask you!!

Joe Cocker done a great version of, I'll get by with a little help from my friends.
Originally sung I think by The Beatles.
Then Wet Wet Wet done a real sh*te version.

East 17 and Gabrielle - If You Ever
The original was recorded by American soul band Shai, though the East 17/Gabrielle version was one of the best songs of the 90s for me.

Although I'm not a big fan of covers, I think most of Westlife's singles are the most popular covers.
